Patients with mutations in NPHS2 (podocin) do not respond to standard steroid treatment of nephrotic syndrome. The impact of an increased urinary albumin excretion in predicting future renal function loss in subjects with diabetes was emphasized already in the 1980s–1990s [1,2]. During these years, the terminology of 'early diabetic ne-phropathy' was introduced to indicate diabetic subjects with microalbuminuria. In this early phase of diabetic ne-phropathy, the glomerular filtration rate (GFR) is still generally well preserved. Early nephropathy contrasts with the later phase of overt nephropathy in which albuminuria increases into the macroalbuminuria range and the GFR falls below 60 ml/min to finally progress to the level of end-stage renal disease (ESRD). The documentation of the early phase of kidney damage with microalbuminuria but still normal GFR helped to better understand the impact of al-buminuria in the loss of renal function in diabetes. Moreover , it led to the demonstration that early intervention by interfering in the renin–angiotensin–aldosterone system (RAAS) [3,4] slows the progression of nephropathy in diabetic patients. Data on the impact of albuminuria on the progressive decline of renal function in non-diabetic renal disease received attention in the 1990s. The findings from large clinical trials, showing the beneficial effect of lowering albuminuria by using agents that interfere with the RAAS to (partially) prevent progression to ESRD [5,6], gave a boost to the concept of focussing the treatment in renal pa

Renal disease, the metabolic syndrome, and cardiovascular disease.

OBJECTIVES To describe the association between renal disease and other features of the metabolic syndrome and its derivative illnesses among Australian Aborigines in remote settings. METHODS Volunteering adults (N = 2019) in four remote communities were screened for risk factors and markers of renal disease, hypertension, and diabetes, and their cross-sectional associations were evaluated. Ra...

Prevalence of diabetic nephropathy in primary care type 2 diabetic patients with hypertension: data from the Korean Epidemiology Study on Hypertension III (KEY III study).

BACKGROUND The prevalence of albuminuria is known to be higher in hypertensive compared to normotensive nondiabetic patients. In addition, systolic blood pressure (BP) is found to be an independent risk factor for albuminuria in type 2 diabetes mellitus (T2DM). Based on these findings, the prevalence of albuminuria is expected to be higher in T2DM with hypertension relative to T2DM without hype...

Prevalence and risk factors of albuminuria and chronic kidney disease in Chinese population with type 2 diabetes and impaired glucose regulation: Shanghai diabetic complications study (SHDCS).

BACKGROUND Diabetes is a major risk factor for the development of kidney disease. We aimed to determine the prevalence of albuminuria and chronic kidney disease (CKD) in Chinese subjects with diabetes and pre-diabetes and the risk factors for kidney disease. METHODS An urban community-based sample of 3714 adults in Shanghai was classified into normal glucose tolerance (NGT), impaired glucose ...

Tubular dysfunction and non-albuminuric renal disease in subjects with type 2 diabetes mellitus.

INTRODUCTION Micro-albuminuria is considered an early marker of glomerular injury in patients with diabetes but it has yet to be determined whether testing for markers of tubular injury can also identify people who are at risk of progressive renal disease. OBJECTIVE To evaluate markers of tubular injury and renal characteristics in a sample of community treated type 2 diabetic subjects. MAT...

Background and purpose: Considering some restrictions of serum low density lipoprotein (LDL) as a marker for Coronary Artery Disease (CAD) risk factor and also the importance of apo B as a signinificant risk factor for CAD, measurement of non-HDL cholesterol has great value as a risk factor for CAD. Non-HDL cholesterol (total cholesterol minus HDL cholesterol) contains all lipoproteins includ...
